The twelfth stage begins at the Tolmin Museum on Mestni trg in Tolmin. From the monument commemorating the Great Tolmin Peasant Revolt (1713), turn left through a small park. Descend past the Tolmin Mountaineering Community building and follow the "Pod brajdo" street-stairs down to Ulica padlih borcev. At the roundabout (with the Hofer shopping center on your left), turn right onto Tigers Square, pass Eurospin, and walk through the park on the left to reach the sports park (featuring a football field and go-kart track).
Walk along the asphalt road; proceed with caution as there are no curbs. Follow the yellow signposts turning right onto a private macadam road toward the Tolmin Sports Kennel Club. To your right is Kozlov rob hill (426 m), and to your left is a meadow frequently used as a landing site for paragliders.
In front of the Kennel Club barracks, transition from the macadam road onto a meadow path. At the edge of the meadow, follow the Juliana Trail signpost left toward Kobarid. This path leads to the Tolmin-Dovje macadam road. After roughly two kilometers, you will reach the left bank of the Soča River and Camp Gabrje. From here, follow the cart track above the riverbank. After 1 km, the path rises sharply to the right, joining the local asphalt road toward Volarje.
Continue for 1.5 km to the settlement of Volarje. Note that the road is narrow and fenced, so stay alert for passing vehicles. Follow signposts through Volarje and continue toward Selišče. After 400 m, the road passes the Church of St. Bric, where the adjacent meadow offers a nice spot for a rest. Pass the First World War military cemetery and enter the village of Kamno.
In Kamno, follow the main road past the primary school. At the road sign for Vrsno/Krn, continue right toward the Juliana Trail signposts at the edge of the village, which direct you onto a left-hand cart track. After 400 m, this becomes a narrow stone-fenced path leading toward Smast. After another kilometer, follow the narrow asphalt road below the Church of St. Lawrence into Smast. Continue through the village on the asphalt road—caution is required here due to traffic and limited space for pedestrians; always walk on the left edge.
Following this panoramic road for 900 m brings you to Ladra. The final 2 km stretch from Ladra leads to the left bank of the Soča and the historic Napoleon Bridge. This section is particularly busy and opaque, so use extreme caution as you approach the bridge, where the road narrows significantly.
